Update for Spring 2006 (mid-May)
Various groups have been discussing the online withdrawal process and the proposed form and generally it has been positively received.
In mid-May, Marvilyn Reyes told me that the Marketing Department has decided that it would be good for us to post it for general information for students as a Withdrawal Information site. They asked me to rework enough to make it absolutely clear that students are not actually withdrawing from a course when they go through this form. It will be posted and appropriate links made from the Admissions and Records portion of the ACC website. Withdrawal Information
The reports generated from the information form will not be as useful as those eventually generated from the actual online withdrawal form since we won't be able to link their answers here to which course they're withdrawing from or other aspects of their scholastic record. However, there may be some interesting information and the reports will enable us to find that. A link will be made here to the report form by Monday May 22. (OK- I didn't make it by May 22, but it's here now!)
(I felt that we had to remove the part about asking them for specific information about the course from which they are withdrawing because if they ever enter that information on the web, some of them will think that they have submitted an online withdrawal form for the course. It is most crucial that we keep them from thinking that until they have actually done it.)

Notes about implementation of proposed online withdrawal form
For those of you in charge of meetings where this will be discussed:
We strongly recommend that you have your committee members go through the online form itself to see how it works.
But if you want those in your meeting to discuss the wording of the various parts, you may prefer to have everyone look at a printed copy of the text. PDF version | Word document
