STEP 1: Check your registration documents.  Each semester several different sections of BIOL 1309 are taught.   These sections are taught during different sessions (16 week, 12 week, 8 week, etc.) and by different instructors.  If you provide the wrong registration information below, you may be dropped from the class for which you registered  and receive no credit for your work!   The registration information you need can be found on your fee receipt.  If you do not have a fee receipt, you can get a print-out of your class schedule from any ACC Admissions Office.  You should contact the Admissions Office if you have any questions about the section(s) in which you are registered.  After carefully reviewing your registration documents, verify the following information by checking the boxes provided.

STEP 4: If you haven't already done so, carefully read the entire Current Course Syllabus and watch all of the streaming video clips. It is recommended that you print out a copy of the Syllabus for future reference.  After reading the Syllabus, check ALL of the following boxes to acknowledge that you understand and agree to follow all the policies in the Course Syllabus.

I understand that I MUST submit a completed copy of this Student Information Sheet or I will be dropped from the course. 

I understand that my professor is Dr. Muzos, where his office is located, and the times for his office hours. I have received his office phone number, web site URL, and email address, and I understand that it is my responsibility to contact him if I have any questions or problems. 

I understand that it is my responsibility to maintain a current email address, to keep it updated in the Blackboard system, and to log in to the Blackboard system for this course at least twice each week to read all of the current and past announcements and to read the Discussion Board postings. 

I understand there are six units in this class (each covering either two or three video programs or episodes). There is also a corresponding study guide which I must obtain. 

I understand that I need an ACC photo ID and a government-issued photo ID to use the ACC Libraries  and Testing Centers

I understand that certain ACC Libraries have checkout copies of the videotapes. I know that each checkout copy contains TWO video episodes. I understand that I must ask for these checkout copies by the UNIT number, not the individual episode numbers. I understand that I may be unable to check out the videotapes if other students have already done so. If this happens, I understand that I may view the videotapes kept in the library, or check out a copy from a different ACC Library. I understand that I am still required to meet the deadlines, even if I am unable to check out videotapes from the ACC Library. 

I understand that the video episodes are NOT broadcast on cable or streamed, and that it is my responsibility to check out the videos from the ACC libraries.

I understand the Testing Center policies and procedures, as outlined in the syllabus, attachments, and Testing Center web site. 

I understand each Unit Exam has TWO versions. I may take EITHER version first and use the other version as a retest.

I understand that I must complete at least ONE exam for EACH of the six units with a grade other than zero. 

I am aware of the unit deadline for each of the six units. 

I understand that there is no point penalty for taking the exams for each unit after the unit Recommended Deadlines, except that no credit will be given for any exams taken after the Absolute Deadline. 

I am aware of the final Absolute Deadlines by which ALL exams and extra credit work must be completed.  I understand that the deadlines occur before the end of the semester and that no credit will be given for work completed after the deadlines. 

I know that I must fill out a Test Request Form when using the Testing Centers. I understand that it is MY responsibility to fill out the form and make sure I receive the correct exam.  I understand that if I take the wrong exam it will not be counted, even if I filled out the Test Request Form correctly. 

I understand the extra credit policy as outlined in the syllabus. 

I understand that it is my responsibility to keep all paperwork for this class until I have received the official course grade from ACC. 

I understand how my grade will be calculated as well as the various options by which I can receive my grades from the professor. 

If I have given an email address, I understand the professor will NOT automatically send my grades by email. I may find  my grades on Blackboard within a reasonable time after taking the exam. If I use a different email address and request grades, I am giving my professor permission to reply with my grades to the address from which I sent my written request. 

I understand and agree to the withdrawal policies as stated in the syllabus. I am responsible for withdrawing myself. 

I agree that it is my responsibility to inform Dr. Muzos of any email, address, or phone number changes. 

I understand and agree to the policy on incomplete grades, as explained in the syllabus. 

I understand that if I do not complete all course work and do not withdraw, I will receive an F for the course. I understand that an F becomes part of my permanent record.

IMPORTANT: Your orientation is not complete until you receive a confirmation email from Dr. Muzos.  Please print out and save a copy of the confirmation email when you receive it; this will be your only proof that you completed the orientation. You should receive your confirmation email within 5 days.  If you do not receive a confirmation email within these time limits, contact your professor immediately; otherwise, you may be dropped from the class.
